Our Aims
Project Summary
Surveys by Carers UK and Healthwatch/British Red Cross show improvements still need to be made to the way patients are discharged from hospital. We are working on a project with Whipps Cross Hospital to understand patient, carer and staff experiences of discharge and review existing information and staff training materials, testing these materials out with carers, identifying key gaps and making improvements where required.
The output of these conversations will be anonymised and shared with staff from Whipps Cross Hospital, after which we will facilitate a workshop focussed on identifying and implementing improvements to the Discharge process.
We will be interviewing a diverse range of Carers, willing to share their experiences of their loved ones being discharged from Whipps Cross, along with their ideas of how hospital discharges can be improved and what support and information Carers require to improve the transition from hospital.
What do we aim to achieve?
• Better guide staff conversations with carers, to better understand the level of information and involvement carers are seeking
• Curate and provide the best information resources that are available for carers, filling any key gaps with new information resources
• Confront the health equalities issues this raises, looking specifically at support for people who speak little or no English
• Use these outputs and our own data to provide stimulus to staff about the importance of supporting carers on discharge, understanding the pressure staff are under.
