Surveys by Carers UK and Healthwatch/British Red Cross show improvements still need to be made to the way patients are discharged from hospital. We are working on a project with Whipps Cross Hospital to understand patient, carer and staff experiences of discharge and review existing information and staff training materials, testing these materials out with carers, identifying key gaps and making improvements where required.
